Per Visit Rates
New Patient Appointments $250, Follow-Ups $150
Starting September 1, 2023
New Patient Appointment (1hr) - $250, includes:
- orders for lab testing
- personalized recommendations for nutrition, activity, stress, and sleep management
- medications that may be helpful in managing your weight and improving your health
Follow-up Appointment (30min) - $150, includes:
- monitoring of response to your personalized treatment plan
- making adjustments as needed in medications, nutrition, activity, mood, sleep and stress management
*patients will be provided with a receipt, for all appointments, which can be submitted to insurance for possible reimbursement (does not apply to Medicare or Medicaid)*
CGM: Optimized Weight Management - $250
Optimize your metabolism using a continuous glucose monitor (CGM) to gain insight into how your body responds to food. This plan includes 2, 45-minute appointments:
- The CGM will be applied during the first appointment, this must be done in person by a practitioner.
- After 10 days, a diabetes and obesity expert will review the data.
- At the second appointment, you will receive recommendations for a personalized meal plan based on these findings.
*This service may be added to any existing follow-up appointment for $79 (existing patients only). Will add 30 min to your appointment.
